I can provide emergency child care.
This is subject to availability and at least two hours notice.

Please provide the following information so I can prepare the written agreement for you to sign and save you time:
Name of child
Date of child's birth
Name of parent(s) or gardian(s)
Home address
Post code
Contact telephone numbers
Dates and times of the required emergency childcare
Emergency contact; other than parent(s) or guardian(s)
Name(s) of person(s) who will bring and collect the child(ren)
Child Doctor's name, address and telephone number.
Further necessary information (such as allergies, especial diet, health problem, childhood illnesses, etc.)
Upon reciept of these information I will contact you to confirm the availability of the place and the amount of fees; as all my fees are paid in advance.
